The designated Child Care Provider is hereby granted full and final authority to:

1. Seek and obtain any necessary medical treatment or care for the Minor Children, including but not limited to consultations, treatments, or hospital services from licensed healthcare professionals.

2. Authorize emergency medical procedures or treatments as may be necessary under the circumstances.

3. Make appropriate decisions related to the Minor Children’s clothing, nutrition, and housing.

4. Provide explanations for school absences and retrieve the Minor Children from school as needed.

5. Sign permission or release forms for school activities, sports, field trips, or similar events.

How to Use This Sample Format

This guide is intended to help you understand and prepare the enclosed Child Care Authorization document. This type of authorization is often overlooked but can be critical in ensuring that your children receive timely medical or emergency care when you are not present. We strongly advise that you attach health insurance cards and any relevant medical information to this form so that the Child Care Provider is fully equipped to respond in emergencies—when time is of the essence and locating paperwork may be impractical.

We recommend storing this document in an accessible location such as the front hallway or entryway table, where the Child Care Provider can easily locate it. It may also be helpful for the Provider to carry an additional copy in their vehicle if they frequently transport your children.

This document should be executed whenever you will be away overnight and especially during extended absences while someone else is responsible for your children’s care.

1. Sign multiple copies and distribute them accordingly. Provide at least one copy to the Child Care Provider and retain additional copies in the recommended accessible locations.

2. This document is practical and valuable. Consider printing several completed versions and keeping them readily available for any upcoming trips or absences.
