How to Use This Sample Format

This guide is intended to assist you in preparing and submitting your request for a divorce certificate.

If you are a Trustee or Executor, it is advisable (though not mandatory) to include a copy of your appointment or authorization along with this request. Doing so may help expedite the process.

Be sure to keep a copy of this request for your own records. This is especially helpful if you need to follow up with a second notice.

We recommend calling the appropriate agency in advance to confirm the following:

Whether a bank or certified check is required instead of a credit card

The total amount due

The correct department or address to which your request should be sent

However, if you are not in urgent need of the document, mailing the request without prior contact may still be effective, allowing the process to proceed at its normal pace.

A divorce certificate is often required for estate administration and other legal or personal matters. If you do not already have a copy on file, it is wise to obtain one and store it with your important documents.
